Wellness Center Use Agreement

The following are guidelines that are to be followed upon receiving a key fob for the West Lyon Wellness Center.

1. The key fob will not be lent to or used by anyone other than the signee to gain access to the Wellness Center.
2. All weights and equipment will be put back before you leave.
3. Any damage or questionable activity will be reported to West Lyon’s Athletic Director within 24 hours of use.
4. This membership only allows for use of the wellness center. Other areas of the facility are off limits without approval.
5. If others are present when departing, you will make sure that they have their own access or are approved supervisors. 

No one under the age of 21 should be left alone or without supervision in the Wellness Center. You can contact Jeremy
