Travel Release Form
This form gives parent(s)/guardian(s) the opportunity to allow someone besides the School District of Cudahy the liability of transportation to and/or from an off campus contest/performance.

Activity Conflict Form
The form is used to create a plan for a student that is in multiple activities at one time, for example 2 sports in the same season and/or a sport and a club/activity in the same season. Should be turned in within 1 week of both activities starting.

Reinstatement of Eligibility Form
This form is needed to regain your academic eligibility once it has been lost by receiving more than 1 F or incomplete for a quarter grade.

Letter Jacket Order Form
This form is used to order a letterman jacket from Mecca, the company we use for our patch awards.

This site is used to order the team and individual pictures from the company we use to take them - VIP (Visual Image Photography).
